Pre-Visit Check-In

The 4-Step Journey

Your barcode is a pre-paid shipping label; it makes the process seamless.

Order & Receive Barcode

Your journey starts when you get an email with your unique barcode. This is your digital ticket.

2️⃣

Identify Your Destination

The barcode's format dictates the lab brand (Quest or LabCorp). They are not interchangeable.

3️⃣

Visit the 'Gate Agent'

Lab staff act like airline gate agents. They scan your barcode into their specific system (**FormFox** for Quest, or LabCorp's internal system) to confirm your pre-paid order.

4️⃣

Automatic Reporting

Once your sample is collected, your part is done. The barcode tracks everything, and results are sent automatically.

One Code, Many Locations

Your barcode is brand-specific (either Quest or LabCorp) but is valid **nationally**. Once you identify your brand, you can visit any of their thousands of locations across the country.

Expired Barcode Help

If the 'Valid Until' date on your email has passed, here are your options:

Employer / Court-Ordered Tests

You **must contact your employer or supervising officer** to request an extension. We cannot approve one without their permission.

We can likely extend it. Please use the SOS tab to text our support team for assistance.

Request a Refund

If an extension isn't possible, you can text our support team to request a refund, subject to our policy.

On-Site Emergency
